Residents must meet the eligibility criteria for assisted living or respite care as defined by Amaris Assisted Living and state regulations.
1.2 Admission Agreement:
A signed admission agreement is required before residency begins. This agreement outlines the services provided, fees, and responsibilities of both parties.
1.3 Assessment:
Amaris Assisted Living reserves the right to conduct an initial assessment of the resident’s needs to determine the appropriate level of care.
2.1 Service Fees:
All fees for services, including room and board, care, and additional amenities, will be outlined in the admission agreement. Fees are subject to change with prior notice.
2.2 Payment Schedule:
Payments are due on the [insert due date, e.g., 1st of each month]. Late payments may incur additional charges as outlined in the admission agreement.
2.3 Refunds:
Refunds are not provided for early termination of residency, except as outlined in the admission agreement or required by law.
3.1 Level of Care:
Amaris Assisted Living provides personalized care plans based on the resident’s needs. Care plans may be adjusted as necessary to ensure the resident’s well-being.
3.2 Medical Services:
While we provide assistance with daily living activities, we are not a medical facility. Residents requiring specialized medical care must arrange for external providers.
3.3 Respite Care:
Respite care services are available for short-term stays. Terms and fees for respite care will be outlined in a separate agreement.
4.1 Compliance with Policies:
Residents and their families are expected to comply with all Amaris Assisted Living policies, including visitation hours, noise restrictions, and safety protocols.
4.2 Personal Belongings:
Residents are responsible for their personal belongings. Amaris Assisted Living is not liable for lost, stolen, or damaged items.
4.3 Health and Safety:
Residents must inform staff of any health concerns or changes in condition. Any behavior that endangers the safety of others may result in termination of residency.
5.1 Visitation Hours:
Visitation hours are from [insert hours, e.g., 9:00 AM to 8:00 PM]. Special arrangements can be made for after-hours visits.
5.2 Guest Policies:
All guests must check in at the front desk and comply with facility policies. Overnight stays are not permitted unless approved in advance.

Amaris Assisted Living respects the privacy of our residents and complies with all applicable privacy laws, including HIPAA.
8.2 Use of Information:
Personal information collected during residency will be used solely for the purpose of providing care and services.
9.1 Mediation:
Any disputes arising from these terms and conditions will first be resolved through mediation.
9.2 Governing Law:
These terms and conditions are governed by the laws of the State of Colorado.
10.1 Changes to Terms:
Amaris Assisted Living reserves the right to update these terms and conditions at any time. Residents and their families will be notified of any significant changes.
By signing the admission agreement, you acknowledge that you have read, understood, and agreed to these terms and conditions.
